Course Description:
Assessments are critical in determining the services and supports an individual receives. Assessments also serve to identify changes and recognize newly acquired skills. The Direct Support Professional has an important role in monitoring the needs and wants of the individual and assuring that services provided support the individual’s goals. The purpose of this course is to provide information describing the role of assessments in helping an individual determine needs and wants and to define the role of the learner in the assessment process.
Course Objective:
1. Explain why assessments are important and how data is collected. 
2. Identify the different types and purposes of assessments and the information gathered by each.
3. Identify the purpose of formal and informal assessments. 
4. Describe how the information gathered by assessments benefits the individual by responding to needs, desires, and interests.
5. Write goals and objectives based on an assessment.
